Tiffany Reiner Obituary, Death – Louisville Metro Police have released new details and identified the officer involved in a shooting that occurred Monday in the Newburg neighborhood, an incident that left two people deceased and prompted an ongoing internal investigation.
According to the Louisville Metro Police Department (LMPD), the officer involved has been identified as Officer Hunter Weaver, who joined the department in 2023. Weaver has reportedly received multiple commendations for professionalism and service during his time on the force.
The event unfolded Monday afternoon on Ridgecrest Road near Rosette Boulevard, when officers responded to reports of a shooting. Upon arrival, officers encountered a man later identified as 20-year-old Frank Mejia Aguilar, who they say was armed and approaching them. According to police accounts, the man did not comply with verbal commands to stop and drop the weapon, leading the officer to open fire.
Aguilar was struck during the encounter and pronounced deceased at the scene. Police stated that the action was taken in response to a perceived immediate threat to officers and bystanders in the area.
Earlier that same day, officers had received calls regarding another shooting nearby, where 45-year-old Tiffany Reiner had been critically injured. She was transported to UofL Health Hospital, where she later passed away from her injuries. Authorities allege that Aguilar was involved in that earlier shooting.
Family members told local news outlet WDRB that Reiner was a mother of three and had been attending a birthday celebration for one of her adult children at the time of the incident. They described her as a devoted mother and a kind-hearted woman whose loss has deeply affected her loved ones.
Deputy Chief Emily McKinley of LMPD confirmed that the department’s Public Integrity Unit has launched a full investigation into the officer-involved shooting, which is standard procedure in such cases. McKinley also stated that body camera footage from the responding officers will be reviewed and made public within 10 business days, in accordance with department policy.
“We understand that transparency is vital in maintaining trust with our community,” McKinley said. “We are committed to conducting a thorough and fair investigation into what occurred.”
Officer Weaver, the officer identified in the shooting, has received at least 10 letters of commendation for his service and one letter of reprimand related to a procedural matter involving secondary employment. He has been placed on administrative leave pending the outcome of the investigation, which is a standard step following incidents involving the use of force.
Residents of the Newburg neighborhood expressed sadness and concern following the events. Several community members spoke to reporters, noting that the area is usually quiet and family-oriented. “It’s heartbreaking,” one neighbor said. “We’re praying for the families affected — both the woman who lost her life and for everyone involved.”
City officials and community leaders have also called for calm and patience as investigators continue to piece together what happened. Louisville Mayor Craig Greenberg issued a brief statement expressing condolences to those affected and reaffirming the city’s commitment to transparency and justice.
Meanwhile, Reiner’s family is remembering her as a loving mother who always put her children first. Friends and relatives have begun sharing memories and tributes online, describing her as warm, generous, and deeply loved by those who knew her.
The investigation remains ongoing, with both the LMPD Public Integrity Unit and the Jefferson County Commonwealth’s Attorney’s Office expected to review all findings once the internal review is complete.
Authorities have encouraged anyone with information, video footage, or witness accounts related to the events on Ridgecrest Road to contact LMPD’s tip line or Crime Stoppers.
As of now, officials have not released any further details regarding the sequence of events or what may have led to the initial shooting involving Reiner. More information is expected to be shared when the body camera footage is made public and the investigation progresses.
